Changes in Cortisol Secretion and Corticosteroid Receptors in COVID-19 and Non COVID-19 Critically Ill Patients with Sepsis/Septic Shock and Scope for Treatment

Sepsis is associated with dysregulated cortisol secretion, leading to abnormal levels of in the blood. In early stages condition, are typically elevated due increased secretion from adrenal glands. However, as disease progresses, may decline impaired function, relative insufficiency. The latter thought be caused by a combination factors, including decreased production corticotropin-releasing hormone (CRH) and adrenocorticotropic (ACTH) hypothalamus pituitary gland, breakdown cortisol. dysregulation sepsis contribute pathophysiology impairing body’s ability mount an appropriate inflammatory response. Given corticosteroid receptors sepsis, there has been considerable interest use steroids treatment. clinical trials have yielded mixed results remains controversial. this review, we will discuss changes critically ill patients sepsis/septic shock. We also make special note COVID-19 patients, who presented recent challenge for ICU management, explore scope administration both non-COVID-19 septic patients.
